To evaluate the visual outcomes and patient satisfaction after unilateral implantation or stepwise bilateral implantation of an apodized diftractive multifocal intraocular lens (IOL). Private practice, Cincinnati, Ohio, USA. Patients requiring cataract surgery in 1 eye were recruited. AcrySof ReSTOR IOLs were implanted in 1 eye of patients with phakic contralateral eyes, patients with contralateral monofocal IOLs, and patients with a contralateral multifocal IOL of the same model. Six months postoperatively, patients were assessed for visual acuity (31.0 cm, 50.0 cm, 4.0 m), contrast sensitivity, and stereopsis. Patients were surveyed for visual disturbances and lifestyle visual quality. Of unilateral multifocal IOL patients, 75% were satisfied with their vision (score 7.3 out of 10) and a majority (56% multifocal-phakic, 65% multifocal-monofocal) achieved spectacle independence. These values were slightly lower than values for bilateral multifocal patients (92% satisfied, 77% spectacle independent), but not at a statistically significant level. Mean overall patient satisfaction was statistically significantly different: 8.9 for bilateral patients versus 7.3 for unilateral patients. Objectively, significant differences favored bilateral implantation over unilateral implantation for stereopsis, uncorrected near visual acuity, and best corrected near and intermediate visual acuities. Contrast sensitivity was similar between groups in most cases. The incidence of halos was lower in unilateral patients (57%) than in bilateral patients (77%), although the difference did not reach statistical significance. Although unilateral implantation of a multifocal IOL provided patients with high levels of spectacle freedom and good visual acuity without compromising contrast sensitivity, overall clinical results favored bilateral implantation.

In case of severe dental erosion, the maxillary anterior teeth are often particularly affected. Restoring such teeth conventionally (ie, crowns) would frequently involve elective endodontic therapy and major additional loss of tooth structure. A novel, minimally invasive approach to restore eroded teeth has been developed and is currently being tested in the form of a prospective clinical trial, termed The Geneva Erosion Study. To avoid crowns, two separate veneers with different paths of insertion have been used to restore the affected anterior maxillary teeth, regardless of clinical crown length and amount of remaining enamel. Purpose We present a study comparing the insertion of central vascular catheter (CVC) and peripherally inserted central catheter (PICC) by an anesthesia nurse at 2 Greek University Hospitals. Methods Eighty patients, aged 20-80 years, were enrolled in the study. Patients were divided into 2 groups. In group A (41 patients), a CVC was inserted in the internal jugular vein. In group B (39 patients), a pressure-injectable PICC was inserted in the basilica vein. Results Correlations between the methods applied, the patients' characteristics, the procedures' characteristics and the overall satisfaction scores for each procedure were examined. The final results show that the patients of group B (PICC method) were more satisfied with the procedure than the patients of group A (CVC method), at the statistical significance level of a=0.01. At academic medical centers, attending rounds (AR) serve to coordinate patient care and educate trainees, yet variably involve patients. To determine the impact of standardized bedside AR on patient satisfaction with rounds. Cluster randomized controlled trial. 500-bed urban, quaternary care hospital. 1200 patients admitted to the medicine service. Teams in the intervention arm received training to adhere to 5 AR practices: 1) pre-rounds huddle; 2) bedside rounds; 3) nurse integration; 4) real-time order entry; 5) whiteboard updates. Control arm teams continued usual rounding practices. Trained observers audited rounds to assess adherence to recommended AR practices and surveyed patients following AR. The outcomes of 55 cases of cubital tunnel syndrome treated by a partial frontal epicondylectomy are presented at a mean follow-up of 38 months follow-up. According to McGowan classification, 25 cases were grade I (45%), 12 grade II (22%) and 18 grade III (33%). The results (Wilson and Krout classification) were excellent or good in 41 patients (75%), fair in nine patients and unchanged in five, without any worsening or recurrence. Total relief was reported in 80% of grade I, 75% of grade II and 66% of grade III patients. Seven painful scars and one persistent 15° elbow extension deficit were the only complications. The satisfaction rate was 93%. This technique preserves bony protection, the blood supply and gliding tissues for the nerve and nerve recovery were comparable to other surgical procedures. Residual pain at the osteotomy site was not a serious problem.

There is growing interest in the provision of trial results to trial participants. However, there are a number of gaps in the research base relating to the closure of clinical trials and feedback of results to participants.The aim of this research was to explore the practice of feeding back trial results to trial participants and to identify best practice in this area. Postal questionnaires were sent to members of the UK National Cancer Research Institute Clinical Studies Groups (NCRI CSG) and to patients over the age of 18 years who completed trial treatment (located in one Cancer Network) during a 16-month period (April 07–July 08).145 NCRI CSG member surveys and 81 patient questionnaires were returned. The vast majority of all respondents supported the idea of offering results to trial participants. However, NCRI members and trial participants differed in their opinions about the timing and method for the provision of results.The results provide an insight into the views of these groups in relation to desire for results and practical aspects of results feedback which should inform further investigations into trial management and the practice of feedback of trial results.

Purpose The aim of our prospective study was to compare patient tolerance of laxative free fecal tagging regimen (LFT) versus traditional cathartic cleansing (TC). Materials and methods 264 patients, at average risk for development of colorectal cancer (105 men and 159 women; mean age 62 years ± 5 SD), underwent 32 rows CT colonography. Patients were alternatively placed into 2 study groups: Group 1 (n = 132) followed TC and Group 2 (n = 132) LFT. TC protocol consisted of no fiber diet and Phospho-lax® 80 mL in 2 L of water the day before imaging. LFT protocol consisted of no fiber diet and ingestion with meals of 30 mL of water-soluble iodinated contrast agent (Gastrografin®) for 2 days before imaging. No frank laxative drugs were administered. All studies were reviewed in a combined fashion, primary 2D followed by 3D endoluminal and dissected views. After the examination all patients were asked to provide a feedback about tolerance to the each bowel preparation. The first 30 patients of each group were also investigated with optical colonoscopy (OC) used as gold standard to confirm our diagnosis (Group 1* and Group 2*). Conclusions LFT reduces discomfort and seems to improve diagnostic accuracy of CTC.
