Migraine and tension-type headaches impose a tremendous economic drain upon the healthcare system. Intravenous and oral niacin has been employed in the treatment of acute and chronic migraine and tension-type headaches, but its use has not become part of contemporary medicine, nor have there been randomized controlled trials further assessing this novel treatment. We aimed to systematically review the evidence of using intravenous and/or oral niacin as a treatment for migraine headaches, tension-type headaches, and for headaches of other etiologic types. We searched English and non-English language articles in the following databases: MEDLINE (1966-February 2004), AMED (1995-February 2004) and Alt HealthWatch (1990-February 2004). Background: Tension-type headache is the most prevalent primary headache type worldwide and is associated with a wide spectrum of disability. Although progress has been made in understanding the complex mechanisms that lead to the pathogenesis of tension-type headache, to date there are no clear-cut markers of what makes tension-type headache unique. Due to a relative lack of research (compared to migraine), the pathophysiology of tension-type headache is not well understood and there are gaps in the epidemiological data, particularly from Australasia.

In spite of all the research dollars spent trying to determine the etiologies of these headaches, the neurology community still has not established a known cause of migraine and tension type headaches. tObjective: The objective of this research is to evaluate the efficacy of manual therapy for tension-typeheadache (TTH) in restoring workers quality of work life, and how work presenteeism affects this relation.Design: This study is a secondary analysis of a factorial, randomized clinical trial on manual therapyinterventions. Altogether, 80 patients (85% women) with TTH and without current symptoms of anyother concomitant disease participated.Interventions: An experienced therapist delivered the treatment: myofascial inhibitory technique (IT),articulatory technique (AT), combined technique (IT and AT), and control group (no treatment).Results: In general, all treatments as compared to our control group had a large effect (f ≥ .69) in theimprovement of participants’ quality of work life. Work presenteeism interacted with TTH treatmenttype’s efficacy on participant’s quality of work life. The inhibitory technique lead to higher reports ofquality of work life than other treatment options only for participants with very low frequency of workpresenteeism. In turn, TTH articulatory treatment techniques resulted in higher reports of quality of worklife for a high to very high work presenteeism frequency.Conclusion: Articulatory manipulation technique is the more efficient treatment to improve quality ofwork life when the frequency of work presenteeism is high. Implications for future research and practiceare discussed.

Most primary headaches are episodic, and most estimates of the heavy disability burden attributed to headache derive from epidemiological data focused on the episodic subtypes of migraine and tension-type headache (TTH). These disorders give rise directly but intermittently to symptom burden. Nevertheless, people with these disorders may not be symptom-free between attacks. We analysed the Eurolight dataset for interictal burden. Eurolight was a cross-sectional survey using modified cluster sampling from the adult population (18-65 years) in 10 countries of the European Union. We used data from nine. To examine the prevalence, impact, and related medication use or overuse of primary chronic daily headache (CDH) among adolescents in a field sample. The authors conducted a two-phase CDH survey of all students from ages 12 to 14 years in five selected middle schools in Taiwan. Subjects with CDH in the past year were identified and interviewed by neurologists. CDH was defined as headache occurring at a frequency of 15 days/month or more, average of 2 hours/day or more, for more than 3 months, and its subtypes were classified on the basis of the International Classification of Headache Disorders, 2nd edition. Of the 7,900 participants, 122 (1.5%) fulfilled the criteria for primary CDH in the past year. Girls had a higher prevalence (2.4%) than boys (0.8%) (p < 0.001). Of the CDH subjects, 88 (72%) could be classified into either chronic tension-type headache (65.6%) or chronic migraine (6.6%). Palpation is widely used to assess muscular sensitivity in clinical settings but still remains a subjective evaluation. This cross-sectional study assessed a newly developed cross-friction algometry making palpation measurable. The objective was to investigate the reliability of pressure pain thresholds obtained using Cross-Friction Algometry (CFA-PPTs) measured at the level of Erector spinae and Gluteus maximus central muscle parts, and to compare the CFA-PPTs between patients with chronic nonspecific low back pain (nCLBP) and matching healthy subjects. Participants: Patients presenting nCLBP to GP's and send into a Pain Center and healthy subjects recruited via university ad valvas & flyers distribution. Outcome measures: 30 patients with nCLBP were measured for cross-friction algometry. Other evaluations consisted of the Visual Analogue Scale (VAS) and the Oswestry Disability Index (ODI). Results: The inter-and intra-reliability were tested and found to be sufficient. The mean CFA-PPT values of the Erector spinae at levels T8, T10, L1 & L3 and the Gluteus maximus of the nCLBP group were significantly lower (p 0.001) when compared to the CFA-PPT values of the healthy group. The greatest difference (À58%) was found at L1 Erector spinae level and at the superior part of the Gluteus maximus measuring point (À59%). Within the group of patients with nCLBP it was surprising to notice that there was no significant correlation between all the reference points measured using CFA-PPTs and the outcomes of the VAS and ODI scores. Conclusions: With the aid of CFA, the importance of local muscular disorder in the lumbar part of the Erector spinae and Gluteus maximus in patients with nCLBP is obviously demonstrated, but also reveals the very large inter-individual differences in muscular fibrosis sensitivity and/

We report the methodology of an epidemiological survey of the prevalences of migraine, tension-type headache and chronic daily headache in Georgia. Medical residents visited adjacent households in Tbilisi to interview a pre-defined target of 100 biologically unrelated subjects. All respondents reporting headache in the previous year, as well as random 20 non-headache controls, were examined by a neurologist. The response rate was 70%. Of 156 respondents, 93 were biologically unrelated and 45 (48%) reported headache in the previous year. Eight subjects fulfilled all IHS criteria for migraine (1-year prevalence 8.6% [95% CI: 2.9–14.3%]), and 13 had probable migraine, meeting all but the criterion for duration. Nineteen had tension-type headache (20.4% [95% CI: 12.2–28.6%]) and five had chronic daily headache (5.4% [95% CI: 1–10.0%]). In comparisons of diagnoses by questionnaire and neurologist (considered the gold standard), sensitivities for the questionnaire of 89% for migraine and 67% for tension-type headache were calculated (overall kappa=0.74).

During a pandemic, excessive workloads cause an increase in physical symptoms, such as tension-type headaches, in medical personnel. Tension-type headache (TTH) is associated with decreased sleep quality which will lead to excessive daytime sleepiness (EDS) and fatigue syndrome. This study aims to determine the relationship between TTH and sleep quality, EDS, and fatigue syndrome in medical personnel during the pandemic. This study is a cross-sectional study conducted on health workers at Sebelas Maret University Hospital, Surakarta, Indonesia in March–August 2020.
