T o examine whether quality of care (QOC) improves when nurse practitioners and pharmacists work with family physicians in community practice and focus their work on patients who are 50 years of age and older and considered to be at risk of experiencing adverse health outcomes. Randomized controlled trial. A family health network with 8 family physicians, 5 nurses, and 11 administrative personnel serving 10 000 patients in a rural area near Ottawa, Ont. Patients 50 years of age and older at risk of experiencing adverse health outcomes (N = 241). At-risk patients were randomly assigned to receive usual care from their family physicians or Anticipatory and Preventive Team Care (APTCare) from a collaborative team composed of their physicians, 1 of 3 nurse practitioners, and a pharmacist. A workshop designed to enhance school personnels' knowledge and understanding of childhood cancer was presented by a multidisciplinary team. Pre- and postworksho assessments were conducted of the participants' attitudes and concerns toward children with cancer, their beliefs concerning how they would react to such children, and their knowledge of medical facts and psychosocial impacts of cancer upon the child and their families. The results are examined in terms of changes in participants' knowledge and attitudes. Implications of these findings are discussed.

The problem of poor compliance/adherence to prescribed treatments is very complex. Health professionals are rarely being asked how they handle the patient's (poor) therapy compliance/adherence. In this study, we examine explicitly the physicians' expectations of their diabetes patients' compliance/adherence. The objectives of our study were: (1) to elicit problems physicians encounter with type 2 diabetes patients' adherence to treatment recommendations; (2) to search for solutions and (3) to discover escape mechanisms in case of frustration. In a descriptive qualitative study, we explored the thoughts and feelings of general practitioners (GPs) on patients' compliance/adherence. Patients with bone metastases may experience pain, fatigue, and decreased mobility. Multiple medications for analgesia are often required, each with attendant side effects. Although palliative-intent radiotherapy (RT) is effective in decreasing pain, additional supportive care interventions may be overlooked. Our objective was to describe the feasibility of multidisciplinary assessment of patients with symptomatic bone metastases attending a dedicated outpatient palliative RT clinic. Consecutive patients referred for RT for painful bone metastases were screened for symptoms and needs relevant to their medications, nutritional intake, activities of daily living, and psychosocial and spiritual concerns from January 1 to December 31, 2007. Consultations by appropriate team members and resulting recommendations were collected prospectively. Patients who received RT were contacted by telephone 4 weeks later to assess symptom outcomes. A total of 106 clinic visits by 82 individual patients occurred. As determined by screening form responses, the clinical Pharmacist, Occupational Therapist, Registered Dietician and Social Worker were consulted to provide assessments and recommendations within the time constraints presented by 1-day palliative RT delivery. In addition to pain relief, significant improvements in tiredness, depression, anxiety, drowsiness and overall well-being were reported at 4 weeks. Systematic screening of this population revealed previously unmet needs, addressed in the form of custom verbal and written recommendations. Multidisciplinary assessment is associated with a high number of recommendations and decreased symptom distress. Our findings lend strong support to the routine assessment by multiple supportive care professionals for patients with advanced cancer being considered for palliative RT.

Neurological disease presents a particular range of problems in pregnancy. The majority of these disorders are preexisting and may require a modification of therapy to ensure that a therapeutic effect is maintained while fetal and neonatal morbidity is avoided. The drug management of epilepsy presents particular challenges in pregnancy. Some neurological conditions occur more frequently in pregnant women, for example compression mononeuropathies, benign intracranial hypertension and cerebral venous and sinus thrombosis, whereas others, such as eclampsia, are unique to pregnancy. The investigation, diagnosis and appropriate management of neurological symptoms and signs in pregnancy require a knowledge of neuroanatomy, physiology of pregnancy and the appropriate application of neuroimaging and neurophysiological testing.

We report a multiple homicide in Angola involving six Portuguese citizens. Immediately after the crime, four bodies were retrieved from the homicide site. Forensic autopsies were allegedly performed by local doctors and three bodies were transferred to Portugal, where they were buried with no extra expertise, presumably identified only by the examinations carried out in Angola. The two remaining bodies, presumably from sub-adults, were only discovered 1 year later. At that time, some bone samples of these two corpses were sent to Portugal. A multidisciplinary team of the National Institute of Legal Medicine (NILM) in Portugal, including forensic anthropologist and pathologists, and forensic geneticist, found out that the bodies were previously misidentified. In fact, the anthropological and genetic examinations on the remains of the two persons showed that they did not belong to a child and an adolescent but instead to two adult victims, at least one of them was supposed to be buried in Portugal since 1 year. The verification of this misunderstanding led to a series of exhumations, of the three victims previously buried in Portugal. In all, the identities were reconstructed and the cause of death could be established in four of them. A multiplicity of severe traumatic cranial injuries were detected, which were, undoubtedly due to a violent death.

Multidisciplinary team care is becoming more popular for complex health issues, including pelvic floor dysfunction. We conducted a retrospective review of the records of 207 patients attending a multidisciplinary pelvic floor dysfunction clinic. A telephone survey was also conducted. The outcomes investigated included conservative versus surgical management, comorbidities, number of specialists seen, and subjective patient feedback. The average patient was 59.9 years old and travelled an average distance of 128.6 km to attend the clinic. Common clinical presentations were urge urinary incontinence (58.9%) and stress urinary incontinence (54.6%). Ninety-three of 178 patients (52.2%) presented with at least two comorbidities. At the time of data collection, 52.2% of patients had opted for surgical management, 27.5% had chosen conservative management, and the remainder remained undecided. RIASSUNTO. Un ampio numero di pazienti ricoverati nei reparti di riabilitazione neuro-motoria sono supportati da caregiver. L'identificazione del carico psico-fisico conseguente all'assistenza o di specifici bisogni del caregiver può aiutare ad indirizzare l'intervento di supporto psicologico rivolto al caregiver stesso. È noto che ansietà e depressione possono essere presenti nel caregiver ed insieme a percezione di stress, di solitudine e di riduzione della attività sociali costituiscono un importante carico psicofisico del caregiver. Obiettivo del presente lavoro è presentare un progetto di supporto psicologico offerto ai caregiver di pazienti ricoverati in riabilitazione neuromotoria, principalmente affetti da ictus. In the fast-growing geriatric population, we are confronted with both osteoporosis, which makes fixation of fractures more and more challenging, and several comorbidities, which are most likely to cause postoperative complications. Several models of shared care for these patients are described, and the goal of our systematic literature research was to point out the differences of the individual models. A systematic electronic database search was performed, identifying articles that evaluate in a multidisciplinary approach the elderly hip fracture patients, including at least a geriatrician and an orthopedic surgeon focused on in-hospital treatment. The different investigations were categorized into four groups defined by the type of intervention. The main outcome parameters were pooled across the studies and weighted by sample size. Out of 656 potentially relevant citations, 21 could be extracted and categorized into four groups. Regarding the main outcome parameters, the group with integrated care could show the lowest in-hospital mortality rate (1.14%), the lowest length of stay (7.39 days), and the lowest mean time to surgery (1.43 days). No clear statement could be found for the medical complication rates and the activities of daily living due to their inhomogeneity when comparing the models. The review of these investigations cannot tell us the best model, but there is a trend toward more recent models using an integrated approach. Integrated care summarizes all the positive features reported in the various investigations like integration of a Geriatrician in the trauma unit, having a multidisciplinary team, prioritizing the geriatric fracture patients, and developing guidelines for the patients’ treatment. Each hospital implementing a special model for geriatric hip fracture patients should collect detailed data about the patients, process of care, and outcomes to be able to participate in audit processes and avoid peerlessness.

This study analyses the development of policies and practice on reducing ESL after 2011, seeking to assess the contribution of the 2011 Council Recommendation on Policies to Reduce Early School Leaving (henceforth the 2011 Recommendation) and associated EU policy instruments to the development of policy, practice and research on ESL across Europe. The study looks at the leverage of EU policy on tackling ESL over the actions taken by key stakeholders at EU level and explores the extent of any influence between MS on their respective approaches to reducing ESL. The study also seeks to assess the added value of EU policy to tackle ESL in the Member States, and investigates the relevance, impact, effectiveness, efficiency and sustainability of the EU policy and tools on reducing ESL.
