Background & Aims: Visceral hypersensitivity was detected in patients with functional gastrointestinal disorders and has been proposed as a biological marker of irritable bowel syndrome (IBS). The purpose of this study was to assess the sensitivity, specificity, and the predictive values of pain thresholds evaluated by rectal distention using an electronic barostat in patients with or without IBS and in control subjects. Methods: Patients were diagnosed according to Rome II criteria. Rectal sensory thresholds were determined in 164 patients (86 IBS patients, 26 painless constipation, 21 functional dyspepsia, and 31 miscellaneous conditions) and in 25 normal controls. All subjects underwent a series of rectal isobaric distentions using an electronic barostat. The bag was progressively distended from 0 to 48 mm Hg and, in response to distention, subjects reported on discomfort or pain. Results: Pain thresholds were lower in IBS patients (30.4 ± 6.7 mm Hg) compared with controls (44.5 ± 5), painless constipated (45.4 ± 5.3), functional dyspepsia (39.4 ± 7.8), and miscellaneous patients (43.2 ± 5.5). At the level of 40 mm Hg, the sensitivity of the rectal barostat to identify IBS patients from normal subjects and non-IBS patients was 95.5% and its specificity was 71.8%. The positive predictive value was 85.4%. The negative predictive value was 90.2%. Conclusions: Lowered rectal pain threshold is a hallmark of IBS patients. Rectal barostat testing is useful to confirm the diagnosis of IBS and to discriminate IBS from other causes of abdominal pain.GASTROENTEROLOGY 2002;122:1771-1777

Sharka, caused by Plum Pox Virus (PPV), is by far the most important infectious disease of peach [P. persica (L.) Batsch] and other Prunus species. The progressive spread of the virus in many important growing areas throughout Europe poses serious issues to the economic sustainability of stone fruit crops, peach in particular. The adoption of internationally agreed-upon rules for diagnostic tests, strain-specific monitoring schemes and spatial–temporal modeling of virus spread, are all essential for a more effective sharka containment. The EU regulations on nursery activity should be modified based on the zone delimitation of PPV presence, limiting open-field production of propagation materials only to virus-free areas. Increasing the efficiency of preventive measures should be augmented by the short-term development of resistant cultivars. Putative sources of resistance/tolerance have been recently identified in peach germplasm, although the majority of novel resistant sources to PPV-M have been found in almond. However, the complexity of introgression from related-species imposes the search for alternative strategies. The use of genetic engineering, particularly RNA interference (RNAi)-based approaches, appears as one of the most promising perspectives to introduce a durable resistance to PPV in peach germplasm, notwithstanding the well-known difficulties of in vitro plant regeneration in this species. In this regard, rootstock transformation to induce RNAi-mediated systemic resistance would avoid the transformation of numerous commercial cultivars, and may alleviate consumer resistance to the use of GM plants.

In this research, experimental applications have been performed to reduce blast induced vibrations in open pit mines. For this purpose, artificial discontinuity zones such as barrier holes and trench were opened in a dragline panel of Seyitomer Lignite Enterprise, Kutahya, Turkey. Peak particle velocities in front of and behind them were measured by seismographs. In this way, their effect against vibration was observed and compared. Barrier holes were opened 24 m deep through 3 parallel lines and aligned at 1 m spacing. On the other hand, the trench had 8 m depth and 4 m width. During the research, 209 measurements were taken belonging to 105 explosions. Twenty-eight explosions were carried out in the barrier holes experiments and 77 explosions were carried out in the trench experiments. A decrease in vibration of 14.3 18.5% was obtained behind the barrier holes while a decrease of 24.8 58.1% was provided by the trench.

Excessive peak inspiratory pressures (PIP) and high tidal volumes (Vt) during manual ventilation can be detrimental to the neonatal lung. We compared the influence of different manual ventilation devices and individual professional experience on the extent of applied Vt and PIP in simulated neonatal resuscitation. One hundred and twenty medical professionals were studied. An intubated mannequin (equivalent to 1.0 kg neonate) was ventilated using two different devices: a self-inflating bag and a T-piece resuscitator. Target value was a PIP of 20 cm H(2)O. Applied PIP and the resulting Vt were recorded continuously using a respiratory function monitor (CO(2)SMO(+), Novametrix, USA). Vt and PIP provision was significantly higher in SI-bags, compared to T-piece devices: median (interquartile range) PIP 25.6 (18.2) cm H(2)O vs 19.7 (3.2) cm H(2)O (p<0.0005), and Vt 5.1(3.2) ml vs Vt 3.6 (0.8) ml (p<0.0005) respectively. To determine the outcomes of vitreoretinal surgery after choroidal tumor biopsy. Retrospective, single-center, consecutive case series. A total of 739 consecutive patients undergoing choroidal tumor biopsy. All subjects who underwent transretinal or transscleral choroidal tumor biopsy for diagnostic or prognostic purposes between May 1993 and May 2013 were identified in our database. We then reviewed patients who subsequently required secondary vitreoretinal surgery for complications arising from such biopsies. The purpose of this study was to compare the diagnostic accuracy of cardiac magnetic resonance (CMR) assessing myocardial viability in patients with chronic left ventricular (LV) dysfunction due to coronary artery disease using 3 techniques: 1) end-diastolic wall thickness (EDWT); 2) low-dose dobutamine (LDD); and 3) contrast delayed enhancement (DE). CMR has been proposed to assess myocardial viability over the past decade. However, the best CMR strategy to evaluate patients being contemplated for revascularization has not yet been determined. Some centers advocate DE CMR due to its high sensitivity to identify scar, whereas others favor the use of LDD CMR for its ability to identify contractile reserve. Despite frequent use of self-reported information to determine pediatric influenza vaccination coverage, little data are available on the validity of parental reporting of their child's influenza vaccination status and on factors affecting its accuracy.We compared parent reported influenza vaccination of children to documented reports of vaccination collected from medical records (the criterion standard) among children aged 6–59 months who presented to selected hospitals, emergency departments, and clinics in three U.S. counties with acute respiratory illness during three influenza seasons (November through May of 2004–2007). Demographic and epidemiologic data were collected from chart reviews and parental surveys.Among 3072 children aged 6–59 months, 47.5% were reported by the parent to have received influenza vaccine and 39.5% of children had medical record verification of influenza vaccination. Sensitivity and specificity of parental reporting was 92.1% and 82.3%, respectively, when compared to the immunization record. However, 17.7% of children whose parents reported vaccination had no influenza vaccination documented in their medical records, and this proportion was even higher at 28.6%, among children with an underlying high-risk medical condition. Greater reporting accuracy was associated with younger age of child (6–23 months vs. 24–59 months), white non-Hispanic race/ethnicity, having health insurance, and having a mother with a college education.Our findings indicate that although parental report of influenza vaccination is fairly reliable (∼76–96%), over reporting by parents often occurs and immunization record review remains the preferable method for determining vaccination status in children.

The organic materials are very important in development of organic light emitting diodes (OLED) in electronic industry. The organic material is based in conjugated polymers of phenylene and vinylene functional group polymers of Poly Phenylene Vinylene (PPV) in thin film form. We have report here the synthesis of PPV precursor polymer in solution-processable route of wessling method. The synthesised PPV precursor polymer converting to PPV conjugated polymer in coating of glass substrate by using one of the thin film preparation method of simple cost effective spin coating technique the coated thin film were annealing to 350 0 C for twenty four hours this time period taken for complete conversion of PPV precursor to PPV conjugated polymer its conductive state. Then we have studied by the structural and vibrational spectroscopy and film optical and luminescence properties were studied in Ultra violet visible spectroscopy.
